1. SAFETY AND CONDUCT
I understand that this membership is contingent upon adherence to the rules and regulations provided by Primitive Jiu-Jitsu Academy, LLC. Members must comply with the rules and regulations entrusted to ensure the safety and satisfaction of all members.

PROHIBITED CONDUCT... includes, but is not limited to:
• Overly aggressive or unsafe behavior
• Failure to follow the instruction of a Primitive Jiu-Jitsu staff
• Inappropriate behavior against Primitive Jiu-Jitsu Academy members or staff

ENFORCED CONDUCT... includes but is not limited to:
• Respect
• Always address black belt instructors as "Professor" and all non-black belt instructors as "Coach."
• Avoid disrupting the class by coaching your children. Please leave any remarks to the instructor after class.
• If you are late for class, wait outside the mat until the professor/coach permits you to join.
• Do not leave class without permission from the professor or coach.
• When techniques are demonstrated, students must sit or stand in good posture.
• All students and participating visitors must wear a clean white, blue, or black gi.
• Always wear your shoes or sandals when you are off the mat.
• If your feet are dirty, wipe them in the bathroom before the start of class.
• Females must wear a rash guard or t-shirt while participating in class.
• All jewelry, piercings, necklaces, and other items should be removed during training.
• Keep fingernails and toenails neatly trimmed.
• Shoes, food, and drinks are not allowed on the mat.
• Try your best not to quit; push yourself.

2. MEDIA AND PHOTOGRAPHY RELEASE:
Primitive Jiu-Jitsu Academy, LLC has my permission to use my or my child’s photograph to promote its business or any of its programs. I understand that the images may be used in, but not limited to: print publications, online publications, presentations, websites, and social media.

3. MEMBERS' RIGHTS TO SERVICES
Membership is contingent on payment. If your account is in suspension, you will not be permitted to attend classes until financial obligations are in order.

4. BUYER’S RIGHT TO FREEZE OR CANCEL THE AGREEMENT
We understand that unexpected life events can get in the way of training. If this occurs, we offer the following options for your convenience. Freezing your recurring payment account when classes are full allows you to keep your membership open until you are ready to come back. It is $15/month and the freezing period can last no longer than 3 months. If you wish to cancel, you may do so by letting an authorized Primitive Jiu-Jitsu staff member know 7 days before your next payment is due. By signing, you understand that cancellation of this membership will not result in a refund.
